What the Winner Receives
The selected recipient will receive up to $2,500 in necessary dental treatment.
Treatment will be determined after a full exam and X-rays and may include services such as:
- Exams
- Cleanings
- Fillings
- Extractions
- Other clinically needed dental care recommended by the dentist
Exact treatment plan is based on clinical findings and may vary by patient.
Giveaway Timeline
- Entry Opens: December 8th
- Entry Closes: December 22nd at 11:59 PM
- Winner Announced: December 23rd
- Winner will be notified directly by phone and email
Who Can Enter?
- Must be 18 or older to enter (or have parent/guardian submit for a minor)
- Must be able to complete the exam and treatment at Revive Dental
- One entry per person
- No purchase necessary
- Void where prohibited
